Job summary

Memorial Hermann is seeking a Clinical Nurse Liaison to perform clinical assessments of patient referrals and present findings to physicians for admission. The role involves participation in process improvement and marketing hospital services to external referral sources and families.

This hybrid position requires RN licensure in Texas, a BSN or related certifications are preferred, and prior case management or rehab experience is advantageous.

Qualifications

• Graduate from an accredited school of professional nursing, vocational nursing or licensed practical nursing.
• Current Texas RN license required; BSN, CRRN, CNRN or CCM preferred.
• Certificate in Basic Life Support required.
• Additional certifications will be required based on discipline and/or nursing unit requirements.
• Minimum of three (3) years nursing experience in rehabilitation, ER, SICU/NICU nursing, or related field.
• Experience in similar position or Case Management strongly preferred.
• Ability to manage multi-task, prioritize work, pay close attention to details and meet deadlines.
• Demonstrate care principles for pediatric and geriatric patients in rehab settings.

Responsibilities

• Conducts clinical assessment of patient referrals and presents assessment to physicians for admission.
• Participates in ongoing process improvement related to admissions.
• Serves as a resource to directors to identify processes hindering referrals to Memorial Hermann.
• Markets services by understanding programs and hospital offerings.
• Participates in hospital marketing programs, events, conferences or in-services.
• Interacts with external referral sources and internal staff to build relationships and maintain confidentiality.
• Promotes quality patient care, safety, cost efficiency, and CQI participation.
