The Hospital for Special Care in Hartford, CT is seeking a Clinical Nurse Leader to provide expert clinical leadership at the point of care for a defined patient population. You will lead care coordination, mentor staff, and collaborate with providers to ensure safe transitions and evidence-based practice.

As the unit-based wound care specialist, you will oversee skin integrity, drive quality initiatives, monitor outcomes, and participate in budget and staffing decisions while promoting

Qualifications

• BSN preferred.
• Active RN license in CT or NLC that authorizes practice in CT.
• Current wound care certification preferred; obtain within six months if not certified.
• Experience collaborating with interdisciplinary teams and providing point-of-care leadership.
• Experience in wound care centers or skin/wound care initiatives preferred.

Responsibilities

• Serve as expert clinician and first point of contact for complex patient care situations.
• Coordinate care with providers and interdisciplinary teams for safe transitions.
• Collect and analyze patient outcomes to improve plans of care.
• Lead unit-based wound care quality initiatives and evidence-based practice.
