Virtua Health seeks an experienced care coordination professional to serve as the single point of contact for referring physicians, patients, and caregivers, guiding them to clinical and supportive services offered within Virtua and the community.

This role facilitates patient appointments with specialty physicians, supports education programs, and helps maintain relationships with key customers across the care continuum.

Qualificações

• Experience in specialty required.
• Problem solving and critical thinking.
• Excellent leadership, organizational and communication skills.
• Ability to work in a self-directed environment and lead teams.
• Strong presentation abilities.
• Ability to implement education programs.

Responsabilidades

• Serves as essential link between patients and care providers; educate patients on diagnosis, options, and resources.
• Facilitate appointments for consults and support services; track patient progress across the care continuum.
• Participate in multidisciplinary planning; collect data and deliver reports on outcomes and improvements.
• Develop and present patient education programs and tools.
• Outreach to maintain positive relationships with physicians and other clinical staff.
