Job summary

Virtua Health is seeking a Registered Nurse to serve as a primary contact for patients, physicians, and caregivers, coordinating access to clinical and supportive care services across Virtua and the community.

The role facilitates appointments with specialty physicians, educates patients about treatment options, and participates in multidisciplinary planning to improve care and outcomes. A valid NJ RN license is required; BSN preferred.

Qualifications

• Experience in specialty required.
• Problem solving, decision making & critical thinking.
• Excellent leadership, organizational, written & verbal communication & interpersonal skills.
• Must be able to work in a self-directed environment, with ability to lead teams.
• Excellent presentation skills.
• Ability to implement professional and community based education programs.
• Database or registry software experience preferred.
• Microsoft Office competency required.

Responsibilities

• Serves as a single point of contact between patients, referring physicians and caregivers to provide resources and access to services.
• Facilitates patient appointments with specialty physicians and care teams.
• Participates in multidisciplinary planning conferences; tracks outcomes and supports improvement efforts.
• Develops and presents patient education programs and tools.
• Outreach to maintain positive relationships with physicians, office staff, diagnostic staff, nurses, radiology and social services.
