Job summary

Virtua Health in Voorhees, NJ seeks an experienced Registered Nurse to coordinate patient care for an assigned group. You will assess, plan, implement and evaluate outcomes with a focus on evidence-based practice and teamwork.

Responsibilities include collaboration with physicians and case management, directing nursing staff, and educating patients and families while upholding high ethical standards. The role is a full-time, day-shift position with 36 hours per week.

Qualifications

• Must be a graduate of an accredited school of nursing and have valid NJ RN License.
• A minimum of 2 years current acute care nursing experience preferred.
• Demonstrates strong communication skills and the ability to coordinate a team of care providers.
• BSN preferred.

Responsibilities

• Coordinates care on the unit/department toward specific outcomes for each patient.
• Prioritizes and coordinates needs of the unit and individual patients.
• Interacts with physicians and case management to coordinate care and develop the plan of care.
• Assigns and evaluates delegated nursing tasks in line with the plan of care.
• Performs patient assessments, development, implementation and evaluation of a plan of care.
• Educates patients and families regarding disease, care needs and safety.
• Maintains professional and departmental competencies.
