Job summary

Allegria Village in Dearborn, MI is seeking an experienced Clinical Navigator (RN/LPN) to guide patients and families from admission through integration into the community. This role blends clinical knowledge with admissions coordination and relationship building.

You will coordinate with hospitals, physicians, discharge planners, and the care team to ensure smooth transitions and high-quality patient experiences.

Qualifications

• Active Michigan RN or LPN license preferred.
• Minimum three years of healthcare admissions, care coordination, discharge planning, case management, or related settings.
• Strong understanding of Medicare, Medicaid, managed care, and admissions processes.
• Excellent communication, customer service, and relationship-building skills.
• Ability to work independently while managing multiple priorities.
• Knowledge of medical terminology and electronic medical records.

Responsibilities

• Navigate patients and families through admission, rehabilitation expectations, and transitions.
• Coordinate with hospitals, social workers, discharge planners, and physicians.
• Assist with insurance verification and authorization processes.
• Support census growth with Admissions and Marketing teams.
• Ensure room readiness and smooth introductions for new admissions.
• Communicate with departments to provide timely updates and support.
