Qualifications

• Coordinate and facilitate admission: referral review, pre-admission screening, and resident education.
• Collaborate with interdisciplinary teams to support care planning and goals.
• Arrange post-discharge services and care transitions with external providers.
• Maintain resident records and provide admin support, data entry, and benefits help.
• Participate in quality assurance and regulatory compliance activities.
