Job summary

The Villages of Hamilton Pointe is seeking a compassionate Clinical Liaison to join our Admissions Team. This role connects prospective residents, families, hospitals, and healthcare providers to support smooth admissions and transitions.

You will review referrals, build relationships with referral sources, coordinate admissions, and represent the facility with professionalism. Nursing background or healthcare experience is preferred.

Qualifications

• Must possess an RN or LPN license with the State of Indiana.
• Knowledge of safe, effective admission practices within Indiana.
• Familiarity with Word and Excel; basic office equipment.

Responsibilities

• Build and maintain relationships with hospitals, discharge planners, case managers, and referral sources.
• Review clinical referrals and assist in determining appropriate placement based on resident care needs.
• Collaborate with the Admissions Team and clinical leadership to ensure a smooth admissions process.
• Communicate with prospective residents and families, providing guidance and support throughout the admissions process.
• Coordinate timely admissions and transitions of care.
• Represent The Villages of Hamilton Pointe with professionalism and a commitment to excellent customer service.
• Support census development through strong community relationships and timely referral follow-up.
