• Active Michigan RN or LPN license preferred.
• Minimum three years of experience in healthcare admissions, care coordination, discharge planning, case management, skilled nursing, or related settings.
• Strong understanding of Medicare, Medicaid, managed care, and healthcare admissions processes.
• Excellent communication, customer service, and relationship-building skills.
• Ability to work independently while managing multiple priorities.
• Knowledge of medical terminology and electronic medical records.

Aufgaben

• Navigate patients and families through admission from hospital to community.
• Coordinate with hospitals, case managers, social workers, discharge planners, and physicians.
• Assist with insurance verification and authorization processes.
• Collaborate with Admissions and Marketing teams to facilitate smooth admissions.
• Ensure room readiness and proper introductions and support for new admissions.
• Communicate admission arrivals to departments to ensure coordination.
• Work with interdisciplinary teams to meet patient needs from day one.
