Qualifications

• Current RN license in good standing
• Minimum 2 years RN experience
• Strong clinical assessment, organization, communication, and problem-solving skills
• Ability to travel throughout the service area for visits and meetings
• Spanish bilingual skills are a plus

Responsibilities

• Lead, inspire, and support field staff to deliver exceptional care
• Provide clinical oversight for Assistive Care State Programs clients
• Conduct client assessments and develop individualized care plans
• Supervise and mentor Home Health Aides and caregivers
• Ensure care delivery aligns with state program requirements and BAYADA standards
• Coordinate with clients, families, caregivers, state case managers, and office partners
• Monitor client conditions and update care plans as needed
• Facilitate orientation and complete competency assessments
• Provide ongoing coaching and clinical support to field staff
• Review documentation for accuracy and compliance

Salary: $97,000–$100,000 annually

Schedule: Full-time, exempt | Monday–Friday | 8:30 a.m.–5:00 p.m. | Combination of office-based leadership and field-based client visits | Occasional travel for client visits, meetings, and community engagement

BAYADA HOME HEALTH CARE is seeking a Registered Nurse (RN) Clinical Manager for our Perth Amboy (PER) Assistive Care State Programs office in Perth Amboy, NJ. In this RN leadership role, you will lead and support a team delivering compassionate, high-quality care to clients throughout Middlesex, Monmouth, Ocean, and Somerset counties.

Our Assistive Care State Programs services help clients receive essential personal care and support at home through Medicaid waiver and other state-funded programs. These services promote safety, independence, and quality of life for clients of all ages while supporting families and caregivers in the community.

What you’ll do

• Lead, inspire, and support field staff to deliver exceptional care
• Provide clinical oversight for Assistive Care State Programs clients throughout the service area
• Conduct client assessments and develop individualized care plans based on client needs and authorized services
• Supervise, mentor, and support Home Health Aides and other caregivers in the field
• Help ensure care delivery aligns with state program requirements, approved service hours, client needs, and BAYADA standards
• Coordinate with clients, families, caregivers, state case managers, managed care organizations, and office partners
• Monitor changes in client condition and help coordinate appropriate updates to care plans and services
• Facilitate orientation for newly hired staff
• Complete competency assessments for new and tenured employees
• Provide coaching, feedback, and ongoing clinical support to field staff
• Review documentation for accuracy, completeness, timeliness, and compliance
• Participate in quality assurance, performance improvement, incident review, and audit-readiness activities
• Partner with office leadership to support quality, compliance, continuity of care, staffing, and service delivery
• Build strong relationships with clients, families, caregivers, referral sources, and community partners
• Conduct field visits, meetings, and community-engagement activities as needed

Required

• Current Registered Nurse (RN) license in good standing
• Minimum of 2 years of RN experience
• Strong clinical assessment, organization, communication, and problem-solving skills
• Ability to provide clinical leadership and support in a community-based care environment
• Ability to manage client needs, documentation, state program requirements, and competing priorities
• Ability to travel throughout the service area for client visits, staff support, meetings, and office needs
• Passion for supporting clients, families, and caregivers in the home setting

Preferred

• Home care or Assistive Care State Programs experience
• Prior supervisory, leadership, or caregiver-management experience
• Experience with Medicaid waiver programs, authorizations, or state-funded services
• Knowledge of state program regulations, care-hour requirements, and documentation standards
• Experience completing caregiver competency assessments or field supervision
• Spanish bilingual skills
• Strong relationship-building skills with clients, families, caregivers, and community partners
