Job summary

Regency Hospitals, a division of Select Medical, is seeking a Clinical Liaison (RN/RT) to advocate for referred patients in the pre-admission process and coordinate care plans with physicians and families. You will facilitate transfers and address barriers to access specialized post-ICU care.

You will work with discharge planners, physicians, and families to plan the continuum of care and ensure smooth pre-admission and transfer processes for medically complex patients in a LTACH setting.

Qualifications

• RN or RT license required.
• Ability to complete clinical assessments of medically complex patients.

Responsibilities

• Plan the best continuum of care after ICU or post-surgical stay by leveraging clinical background, payer knowledge, and problem-solving skills.
• Conduct clinical assessments of referred patients and counsel families to help them make informed care decisions.
• Collaborate with hospital case managers and physicians to streamline pre-admission and transfer processes, removing payer-related and operational barriers.

Qualifications: Registered Nurse (RN) or Respiratory Therapist (RT) License (Other designations like LPN, PT, OT, etc., upon further review) - Ability to complete clinical assessments of medically complex patients.

A day in the life

you'll advocate for referred patients in the pre-admission process by collaborating with clinical teams to assess patient needs. You will serve as a resource for healthcare professionals through fostering relationships within medical facilities. With clinical expertise and problem-solving skills, you'll also coordinate care plans with physicians and families. Additionally, you will facilitate patient transfers and address barriers to accessing specialized post-ICU care.

• Full-time: Monday-Friday 8-5 PM
• Competitive Salary ($70,000 - $90,000 depending on experience) plus Uncapped Monthly Bonus Incentives

Key Responsibilities

• You will work with physicians, discharge planners, and patients' families to plan the best continuum of care after an ICU or post-surgical stay by utilizing your strong clinical background, payer knowledge, and problem-solving skills.

Required Skills

What we're looking for in a Clinical Liaison

• Registered Nurse (RN) or Respiratory Therapist (RT) License.

• Ability to complete clinical assessments of medically complex patients.

• Demonstrated success in patient advocacy.
• Superior communication and relationship-building skills.
• Prior healthcare marketing or business development experience is preferred, but extensive training is provided on this piece.
