Job summary

Select Specialty Hospital in Greensboro is seeking a Clinical Liaison (RN or RT/RRT) to advocate for referred patients during pre-admission, coordinating with clinical teams to assess needs and develop care plans. You will support physicians, families, and case managers in facilitating smooth transfers to our post-ICU level of care.

The role emphasizes clinical expertise, payer knowledge, and problem-solving to minimize barriers to access specialized post-ICU care.

Qualifications

• Registered Nurse (RN) or Respiratory Therapist (RT) license; other designations may be considered.

Responsibilities

• Plan continuum of care after ICU or post-surgical stay with clinicians, discharge planners, and families.
• Counsel patients' families to support informed care decisions.
• Collaborate with hospital case managers and physicians to remove payer/operational barriers for pre-admission and transfers.

A day in the life

You’ll advocate for referred patients in the pre-admission process by collaborating with clinical teams to assess patient needs. You will serve as a resource for healthcare professionals through fostering relationships within medical facilities. With clinical expertise and problem-solving skills, you’ll also coordinate care plans with physicians and families. Additionally, you will facilitate patient transfers and address barriers to accessing specialized post-ICU care.

• Full-time: Monday-Friday 8-5 PM

Key Responsibilities

• You will work with physicians, discharge planners, and patients' families to plan the best continuum of care after an ICU or post-surgical stay by utilizing your strong clinical background, payer knowledge, and problem-solving skills.
• Perform clinical assessments of referred patients, then counsel with patients' families to assist them in making the best care decisions for their loved one.
• Partner with hospital case managers and physicians to create a smooth pre-admission and transfer process through removing payer-related and operational barriers for patients getting into our highly effective level of care.

Required Skills

• Registered Nurse (RN) or Respiratory Therapist (RT) License. Will consider other designations like LPN, PT, OT, etc., upon further review.
• Ability to complete clinical assessments of medically complex patients.

Preferred skills that will make you a successful Clinical Liaison:

• Demonstrated success in patient advocacy.
• Superior communication and relationship-building skills.
• Prior healthcare marketing or business development experience is preferred, but extensive training is provided on this piece.
