Select Specialty Hospital in the LTACH segment seeks a Clinical Liaison (RN / RT) to advocate for referred patients during pre-admission, coordinate with clinical teams, and work with families and physicians on care plans. You will help remove barriers to accessing post-ICU care and coordinate transfers.

With clinical expertise and problem-solving skills, you’ll also coordinate care plans with physicians and families. Additionally, you will facilitate patient transfers and address barriers to accessing specialized post-ICU care.
