Job summary

Select Specialty Hospital’s Critical Illness Recovery Hospital (LTACH) is seeking a Per-Diem Clinical Liaison (RN / RRT) to advocate for referred patients during pre-admission, coordinate care plans with physicians and families, and facilitate smooth transfers from ICU or post-surgical care.

The role collaborates with discharge planners, physicians, and families to determine the best continuum of care. The Per‑Diem liaison covers some weekends, PTO, and medical leave for full‑time employees.

Qualifications

• Current and valid state licensure or eligibility for state licensure as a Registered Nurse or Respiratory Therapist.
• Two years of related clinical healthcare experience is preferred.
• Experience with the preadmission process and relationship building in rehabilitation or post-acute care settings is preferred.
• Sales and marketing experience in healthcare is preferred.
• Demonstrated computer proficiency.
• Ability to exercise sound judgment regarding confidential healthcare information.

Responsibilities

• Plan the best continuum of care after ICU or post-surgical stays by leveraging clinical background, payer knowledge, and problem-solving skills.
• Perform clinical assessments of referred patients, then counsel families to support informed care decisions for loved ones.

A day in the life

You’ll advocate for referred patients in the pre‑admission process by collaborating with clinical teams to assess patient needs. You will serve as a resource for healthcare professionals through fostering relationships within medical facilities. With clinical expertise and problem‑solving skills, you’ll also coordinate care plans with physicians and families. Additionally, you will facilitate patient transfers and address barriers to accessing specialized post‑ICU care.

Key Responsibilities

• You will work with physicians, discharge planners, and patients' families to plan the best continuum of care after an ICU or post‑surgical stay by utilizing your strong clinical background, payer knowledge, and problem‑solving skills.
• Perform clinical assessments of referred patients, then counsel with patients' families to assist them in making the best care decisions for their loved one.
• Partner with hospital case managers and physicians to create a smooth pre‑admission and transfer process by removing payer‑related and operational barriers for patients getting into our highly effective level of care.

Minimum Qualifications

• Current and valid state licensure or eligibility for state licensure as a Registered Nurse or Respiratory Therapist.

Preferred Experience

• Two years of related experience working in a clinical healthcare setting or previous experience working in a clinical liaison role are preferred.
• Demonstrated competency with the preadmission process, including assessments and relationship building in a rehabilitation setting or in another post‑acute care setting, is preferred.
• Proven track record in sales and marketing with at least one year of experience in a related position preferred.
• Demonstrated computer proficiency.
• Must exercise sound judgment in handling the professional/confidential nature of health care sales.
