Baylor Scott & White Inpatient Rehabilitation Hospital in Frisco, TX, is seeking a Clinical Liaison (RN, OT, PT, or SLP) to advocate for referred patients during pre-admission, coordinate care with physicians and families, and facilitate transfers to post-acute rehab services.

You will build relationships with hospitals, educate referral sources, and support sales/marketing efforts to grow the rehab services within this joint venture with Select Medical.

Qualifikationen

• Current and valid state licensure or eligibility for state licensure as RN, OT, PT, or SLP.
• Multi-state licensure required based on geographical territory.
• Two years related clinical healthcare experience preferred.

• Lead relationships with case managers and clinicians in assigned hospitals.
• Educate staff, patients and families about rehabilitation services.
• Coordinate care plans with physicians and families in the rehab pathway.
• Facilitate patient transfers and address barriers to accessing post-acute care.
• Develop expansion strategies with VP/Director of Business Development to grow market share.
