Job summary

Baylor Scott & White Institute for Rehabilitation in Dallas, TX seeks a Clinical Liaison (RN, PT, OT, or SLP) to advocate for referred patients during pre-admission, collaborating with clinical teams to assess needs and coordinate care across physicians and families.

You will build relationships with case managers and referring hospitals, educate staff and patients on rehabilitation services, and help manage the referral process to our inpatient facility.

Qualifications

• Current and valid state licensure or eligibility for state licensure as a Registered Nurse, Physical Therapist, Occupational Therapist, or Speech Language Pathologist; multi-state licensure required based on territory.

Responsibilities

• Advocate for referred patients in pre-admission by coordinating with clinical teams to assess patient needs.
• Serve as a resource for healthcare professionals by fostering relationships within medical facilities.
• Coordinate care plans with physicians and families; facilitate patient transfers and address barriers to accessing post-acute care.
• Develop relationships with case managers and physicians in acute care hospitals; educate staff, patients, and families about rehabilitation services.
