Job summary

Baylor Scott & White Health is seeking a Clinical Liaison to build relationships with referral sources and evaluate patients for post-acute rehab services. The liaison collaborates with physicians and nurses to ensure Medicare guidelines are followed and documentation is maintained throughout the rehab stay.

The role requires coordinating admissions, educating patients and families, and promoting inpatient rehabilitation programs.

Qualifications

• 2 years of experience in care coordination or rehab/bridging across care settings.
• RN licensure or related clinical licensure preferred.
• Knowledge of Medicare guidelines and inpatient rehab processes.

Responsibilities

• Perform pre-admission assessments to determine rehab eligibility.
• Coordinate admissions with referral sources, patients, and rehab team.
• Educate patients and families on rehab services and processes.
• Develop and maintain relationships with referral sources and external partners.
• Support market share growth for rehab services in the assigned hospital.
• Ensure Medicare compliance and proper IRF documentation (IRF-PAI).
• Provide exceptional customer service and timely referral processing.

Job Summary

The Clinical Liaison builds relationships with current and potential referral sources. The liaison evaluates patients for post-acute care services. The liaison performs pre-admission assessments to establish if patients qualify for the inpatient rehabilitation center. The liaison collaborates with physician and nurse managers to ensure compliance with Medicare guidelines. Proper documentation is maintained both on pre-admission and throughout the patient's rehab stay.

Essential Functions Of The Role

• Completes preadmission assessments timely, thoroughly, and with sufficient detail to justify a rehab admission.
• Coordinates admission process with referral source, patient/family and rehab team through effective communication and documentation.
• Educates potential patients and families, as well as referral sources on inpatient rehabilitation and the benefits of the program.
• Develops and maintains positive business relationships with key referral sources.
• Develops and implements strategies to grow market share in the core business line in the assigned hospital.
• Promotes BSW services and programs to the external community. Is available to assist with health fairs and educational programs as needed.
• Works closely with service line leadership and Clinical Liaisons. Promotes rehab services by participating in educational programs, meet and greets, and interdisciplinary meetings in hospitals or other care levels.
• Ensures compliance with Medicare guidelines and proper IRF documentation both on pre-admission and throughout the patient's rehab stay.
• Understanding of the IRF-PAI (Inpatient Rehabilitation Facility-Patient Assessment Instrument).
• Provides exceptional customer service by ensuring patient referrals are managed and completed in a timely manner.

Key Success Factors

• Knowledge of care management and resource utilization management.
• Knowledge of current Medicare PPS guidelines and regulations related to inpatient rehab compliance and IRF-PAI management.
• Knowledge and understanding of nursing and patient care standards and procedures.
• Know laws, rules, and regulations. Understand standards and guidelines of certifying and accrediting bodies. Follow hospital and department/unit standards, protocols, policies, and procedures for nursing care in the assigned area.
• Know medical terms, health promotion, risk reduction, and disease prevention. Understand medications, common dosages, effects, and possible adverse reactions.
• Knowledge of medical and professional nursing ethics and patient privacy rights.
• Must be able to communicate thoughts clearly, both verbally and in writing.
• Skills related to promoting and marketing a service line.
• Ability to work collaboratively with both internal and external referral sources and external partners and organizations.
• Must have critical thinking and problem-solving skills.
• General computer skills include Microsoft Office, information security, scheduling and payroll systems, electronic medical documentation, and email.
