Job summary

KeystoneCare in Philadelphia is seeking a dedicated Clinical Liaison Registered Nurse (RN) to advocate for patients in acute care until they are safely discharged home. This role focuses on coordinating care, guiding families through options, and ensuring a smooth discharge process.

You will build relationships with case managers and social workers to generate referrals for home health, palliative and hospice services, waivers, and private duty care, while maintaining clinical oversight and

Qualifications

• Valid RN license in Pennsylvania.
• 3+ years of experience in clinical care coordinator, liaison, or case manager role preferred.
• Self-motivated with strong time management and communication skills.
• Clinical knowledge of diseases common to Home Health and Hospice.
• Driver's license and a reliable vehicle required.
• Proof of Influenza (Flu) Vaccination or an approved Medical or Religious exemption.

Responsibilities

• Generate referrals for home health, palliative care, hospice, waiver, and private duty services by building relationships with case managers and social workers.
• Assist patients and families through discharge planning and transition of care.
• Ensure eligibility verification for care and guide patients through care options.

The Clinical Liaison RN is responsible for generating referrals for home health, palliative care, hospice, waiver, and private duty services by building strong relationships with case managers and social workers. In addition to meeting with patients and their families, you will ensure a seamless transition of care throughout the discharge process, guiding them through their options and verifying eligibility for care.

Job Requirements:

• Valid RN license in the state of Pennsylvania.
• 3+ years of experience in a clinical care coordinator, liaison, or case manager role preferred.
