Primary Job Responsibilities

• Supervise and support nursing and clinical staff, ensuring quality and consistency of patient care
• Review and approve patient care plans, documentation, and clinical records for accuracy and compliance
• Provide coaching, training, and performance feedback to clinical team members
• Ensure compliance with state, federal, and accreditation standards (e.g., Medicare, CHAP)
• Assist with case management, scheduling coordination, and patient assignments as needed
• Collaborate with interdisciplinary teams to ensure coordinated and effective patient care
• Participate in quality improvement initiatives and clinical audits
• Serve as a clinical resource for staff, patients, and families
• Support onboarding and orientation of new clinical staff
• Accepts direction from CEO and VP of Clinical Services
• Maintain confidentiality with regard to patient information
• Performs other duties as assigned

Requirements

• 3+ years' experience with a healthcare organization; preferably in acute care, home health or hospice environment
• RN License
• Must be able to multi-task in a fast-paced environment and communicate effectively
• Excellent customer service skills and proven ability to work with healthcare professionals, patients and their families responding to time-sensitive needs
• Excellent data entry skills
• Dedicated self-starter who can work both independently as well as collaboratively as part of a team of healthcare professionals
• Proven technology skills and experience using an electronic documentation system
• Proficient in Microsoft computer products including Word, Excel and Outlook

Qualifications

• Active and unrestricted RN license in Pennsylvania (required)
• Minimum 2-5 years of clinical experience in home health, hospice, or palliative care
• Prior supervisory or leadership experience preferred
• Strong knowledge of Medicare/Medicaid regulations and documentation requirements
• Excellent communication, leadership, and organizational skills
• Ability to manage multiple priorities in a fast-paced environment
