A leading home health and hospice provider seeks an energetic and motivated Registered Nurse (RN) to serve as a Hospital Clinical Liaison. This role focuses on generating referrals, ensuring smooth transitions of care, and collaborating with healthcare professionals. Candidates should have a valid RN license, clinical knowledge in home health, and strong communication skills, along with the ability to manage patient assessments effectively.

Qualifications

• Valid RN license in Pennsylvania required.
• Preferred 3+ years of clinical coordination experience.
• Must have clinical knowledge common to Home Health and Hospice.

We are seeking an energetic, highly motivated, results driven Registered Nurse (RN) who serves as an advocate for patients within an acute care setting until discharged to home. The Hospital Clinical Liaison is responsible for generating referrals for home health, palliative care, hospice, waiver and private duty services by building relationships with case managers and social workers. They will meet with patients and their families to ensure a smooth transition of care through the discharge process. The Hospital Clinical Nurse Liaison will be responsible to explain care options to patients and their families. They will also review the patient's medical records and verify eligibility for care.

Primary Job Responsibilities:

• Serves as the primary referral contact between assigned hospital partners and KeystoneCare
• Establishes and fosters professional relationships with case managers, discharge planners, and social workers
• Performs clinical assessment of potential referrals to determine appropriateness against eligibility criteria
• Ensures a smooth transition of care from hospital to home by assessing and communicating all patient needs prior to discharge
• Makes regular, planned visits to assigned facilities and is continually available by phone during business hours
• Works to build the brands of Home Health, Palliative Care, Hospice, and Private Duty services within the designated market
• Achieves or exceeds targets for referral volume and growth.
• Demonstrates knowledge of all assigned hospitals and stays abreast of changes and trends in the marketplace
  
  
  

Requirements

Job Requirements:

• Valid RN license with the state of Pennsylvania
• 3+ years experience as a clinical care coordinator, liaison, or case manager preferred
• Self motivated with excellent time management and communication skills
• Must have clinical knowledge of disease entities common to Home Health and Hospice
• Must have a valid Driver's License and a reliable vehicle
• Proof of COVID-19 Vaccination or an approved Medical or Religious exemption
• Proof of Influenza (Flu) Vaccination or an approved Medical or Religious exemption
