Qualifications

  • Active RN license in Pennsylvania.
  • Minimum 3 years experience as a clinical care coordinator, liaison, or case manager.
  • Valid driver’s license and reliable vehicle.
  • Strong communication and time-management skills.

Responsibilities

  • Serve as the primary clinical liaison between assigned hospital partners and the organization.
  • Build and maintain strong professional relationships with case managers, discharge planners, physicians, and social workers.
  • Conduct clinical evaluations of patients to determine eligibility for hospice and related services.
  • Collaborate with care teams to ensure smooth and timely transitions from hospital to home or appropriate care setting.
  • Educate patients and families on hospice, palliative care, and support service options, ensuring informed decision-making.
  • Review and interpret medical records to confirm clinical appropriateness and compliance with eligibility criteria.
  • Maintain regular on-site presence at assigned facilities and remain accessible for real-time coordination.
  • Support growth by facilitating referrals and strengthening provider relationships.
  • Stay informed on hospital processes, patient needs, and industry trends impacting care transitions.
