Nashua Post Acute and Healthcare Center is seeking a Clinical Liaison to build strong relationships with hospitals and healthcare partners, facilitating seamless care transitions and referrals. The role requires travel between facilities and strong communication within a multidisciplinary team.

The ideal candidate holds an RN, LPN, or LSW license (preferred) with an associate degree (bachelor's preferred) and a valid driver's license.

Responsibilities for Clinical Liaison:

• Build and maintain strong, professional relationships with medical professionals, hospitals, and other healthcare facilities and organizations to support effective care coordination and referrals.
• Promote the organization's philosophy, mission, and objectives both internally and externally, serving as a trusted representative of our care standards and values.
• Attend and actively participate in facility meetings, calls, and in-services to support collaboration, education, and strong communication across care settings.
• Demonstrate honesty, integrity, and professionalism in all interactions, and ensure the respectful handling of patient information and hospital property.
• Maintain accurate, up-to-date referral-tracking logs to support efficient admissions and care coordination.
• Prepare and distribute timely end-of-day reports to department leaders to ensure clear communication and operational alignment.
• Collaborate closely with interdisciplinary teams to support smooth transitions of care and ensure resident needs are clearly communicated and addressed.

Qualifications for Clinical Liaison:

• Valid driver's license required, as travel between facilities and partner organizations is essential.
• Current RN, LPN, or LSW licensure preferred.
• Associate's degree in a related field required; Bachelor's degree preferred.
• Strong communication and relationship-building skills with the ability to engage effectively with healthcare professionals and community partners.
• Ability to work collaboratively with multiple residents, patients, and care teams in a fast-paced healthcare environment.
• Strong organizational skills with attention to detail and accuracy in documentation and reporting.
