Job summary

Marquis Health Consulting Services partners with Nashua Post Acute and Healthcare Center to recruit a Clinical Liaison. This full-time role emphasizes building clinical relationships, coordinating care transitions, and ensuring effective communication across hospitals and partner facilities.

The successful candidate will travel between partner sites, maintain referral logs, and represent our standards of patient-centered care with integrity and professionalism.

Qualifications

• Valid driver's license required; travel between facilities is essential.
• RN, LPN, or LSW licensure preferred.
• Associate's degree required; Bachelor's degree preferred.
• Strong communication and relationship-building with healthcare professionals.
• Ability to work with multiple residents and care teams in a fast-paced environment.
• Strong organizational skills and attention to documentation.

Responsibilities

• Build and maintain strong, professional relationships with medical professionals, hospitals, and other healthcare facilities and organizations to support effective care coordination and referrals.
• Promote the organization's philosophy, mission, and objectives both internally and externally, serving as a trusted representative of our care standards and values.
• Attend and actively participate in facility meetings, calls, and in-services to support collaboration, education, and strong communication across care settings.
• Demonstrate honesty, integrity, and professionalism in all interactions, and ensure the respectful handling of patient information and hospital property.
• Maintain accurate, up-to-date referral-tracking logs to support efficient admissions and care coordination.
• Prepare and distribute timely end-of-day reports to department leaders to ensure clear communication and operational alignment.
• Collaborate closely with interdisciplinary teams to support smooth transitions of care and ensure resident needs are clearly communicated and addressed.
