Job summary

Marquis Health Consulting Services is seeking a Clinical Liaison on a full-time basis in the United States to facilitate patient-centered care transitions and strong collaboration across care settings.

The role requires building relationships with hospitals and healthcare partners, travel between facilities, and maintaining accurate referral logs. RN/LPN/LSW licensure is preferred, with an associate degree required and a bachelor's degree preferred.

Responsibilities

• Build and maintain strong professional relationships with medical professionals, hospitals, and healthcare facilities to support referrals.
• Promote Marquis Health Consulting Services' mission and care standards to internal and external partners.
• Attend facility meetings, calls, and trainings to support collaboration and communication across care settings.
• Demonstrate professionalism and protect patient information and hospital property.
• Maintain updated referral-tracking logs to support admissions and coordination.
• Prepare and distribute end-of-day reports to department leaders for alignment.
• Collaborate with interdisciplinary teams to ensure smooth transitions of care.

Responsibilities for Clinical Liaison:

• Build and maintain strong, professional relationships with medical professionals, hospitals, and other healthcare facilities and organizations to support effective care coordination and referrals.

• Promote the organization’s philosophy, mission, and objectives both internally and externally, serving as a trusted representative of our care standards and values.

• Attend and actively participate in facility meetings, calls, and in-services to support collaboration, education, and strong communication across care settings.

• Demonstrate honesty, integrity, and professionalism in all interactions, and ensure the respectful handling of patient information and hospital property.

• Maintain accurate, up-to-date referral-tracking logs to support efficient admissions and care coordination.

• Prepare and distribute timely end-of-day reports to department leaders to ensure clear communication and operational alignment.

• Collaborate closely with interdisciplinary teams to support smooth transitions of care and ensure resident needs are clearly communicated and addressed.

Qualifications for Clinical Liaison:

• Valid driver’s license required, as travel between facilities and partner organizations is essential.

• Current RN, LPN, or LSW licensure preferred.

• Associate’s degree in a related field required; Bachelor’s degree preferred.

• Strong communication and relationship-building skills with the ability to engage effectively with healthcare professionals and community partners.

• Ability to work collaboratively with multiple residents, patients, and care teams in a fast-paced healthcare environment.

• Strong organizational skills with attention to detail and accuracy in documentation and reporting.
