Qualifications

• Previous experience as a Clinical Liaison preferred.
• Experience in a skilled nursing facility required.
• Working knowledge of SNF admissions, referrals, and clinical protocols.

Responsibilities

• Collaborate with hospitals, physicians, families, and interdisciplinary teams to coordinate patient admissions.
• Guide referrals through the skilled nursing facility admissions process.
• Support and ensure adherence to clinical protocols and resident care plans.
