Job summary

Marquis Health Consulting Services is seeking a Clinical Liaison for a full-time role. This position focuses on building strong relationships with hospitals and healthcare partners to ensure smooth transitions and high-quality patient care.

Travel between facilities is essential, and licensure along with related degrees are preferred. The Clinical Liaison will work with interdisciplinary teams to coordinate referrals, communicate care plans, and maintain accurate documentation.

Qualifications

• Valid driver’s license required; travel between facilities and partner organizations is essential.
• Current RN, LPN, or LSW licensure preferred.
• Associate’s degree in a related field required; Bachelor’s degree preferred.
• Strong communication and relationship-building skills with healthcare professionals and community partners.
• Ability to work collaboratively with residents, patients, and care teams in a fast-paced healthcare environment.
• Strong organizational skills with attention to detail and accuracy in documentation and reporting.

Responsibilities

• Build and maintain strong, professional relationships with medical professionals, hospitals, and other healthcare facilities and organizations to support effective care coordination and referrals.

• Maintain accurate, up-to-date referral-tracking logs to support efficient admissions and care coordination.
• Prepare and distribute timely end-of-day reports to department leaders to ensure clear communication and operational alignment.
• Collaborate closely with interdisciplinary teams to support smooth transitions of care and ensure resident needs are clearly communicated and addressed.
