Qualifications

• Active LPN or RN license required or preferred.
• Clinical background in skilled nursing, post-acute care, discharge planning, or hospital case management helpful.
• Excellent communication, organization, and relationship‑building skills are essential.

Responsibilities

• Build and maintain relationships with hospitals, physicians, discharge planners, case managers, and referral sources in Billings and Laurel markets.
• Evaluate referrals and assist with clinical screenings and pre‑admission assessments.
• Coordinate admissions to ensure timely transitions into skilled nursing communities.
• Serve as a trusted clinical resource for patients, families, and referral partners.
• Partner with facility leadership and admissions teams to meet census and resident care goals.
• Track referral activity and participate in business development initiatives.

What You'll Do

What We're Looking For

• Active LPN or RN license preferred
• Clinical background in skilled nursing, post‑acute care, hospital case management, discharge planning, or healthcare marketing strongly preferred
• Excellent communication, organization, and relationship‑building skills
• Self‑motivated with the ability to work independently across multiple locations
• Willingness to travel regularly throughout the Billings and Laurel, Montana markets
• Must reside within a reasonable commuting distance of the territory
