Job summary

Evergreen Healthcare Group in Tacoma, WA seeks a Clinical Liaison to connect hospitals, families, and our two skilled nursing facilities—Alaska Gardens and Heartwood Extended Care. This regional role focuses on admissions and post-acute transitions, fostering trust and ensuring quality care.

Active LPN or RN license preferred, strong communication, and willingness to travel between facilities are required. Starting salary is $110,000 per year with mileage reimbursement and comprehensive private

Qualifications

• Active LPN or RN license (preferred) or clinical background in healthcare.
• Experience in skilled nursing, post-acute, or discharge planning preferred.
• Excellent communication, organization and relationship-building skills.

Responsibilities

• Develop and maintain relationships with hospitals, discharge planners and case managers.
• Evaluate referrals and assist with pre-admission assessments.
• Coordinate admissions and ensure smooth transitions from acute to post-acute care.
• Serve as a clinical resource and liaison to families and referral sources.
• Collaborate with facility teams to align care plans and census initiatives.
• Track referral activity and support census development.

Are you a nurse or clinically experienced healthcare professional ready to make a meaningful impact beyond bedside care? We’re looking for a compassionate, relationship-driven Clinical Liaison to join the Evergreen Healthcare Group team - serving two skilled nursing facilities in the Tacoma market: Alaska Gardens and Heartwood Extended Care.

In this regional role, you’ll serve as the key connection between hospitals, families, and our facilities - helping patients transition to post-acute care with confidence and clarity.

You’ll play a critical role in coordinating seamless admissions, building trust with healthcare partners, and ensuring patients receive the care they need in a setting that supports healing and quality of life.

Key Responsibilities:

• Develop and maintain relationships with hospitals, physicians, discharge planners, and case managers
• Evaluate patient referrals and assist with clinical screenings and pre-admission assessments
• Coordinate admissions and ensure timely, efficient transitions from acute to post-acute care
• Serve as a clinical resource and liaison to families and referral sources
• Collaborate with internal facility teams to align care plans and meet resident needs
• Track referral activity and participate in census development initiatives

Qualifications:

• Active LPN or RN license (preferred) or clinical background in healthcare, case management, or marketing
• Experience in skilled nursing, post-acute, or hospital-based discharge planning preferred
• Excellent communication, organization, and relationship-building skills
• Ability to work independently and travel regularly between facilities
• Must reside within commuting distance of Tacoma, WA
