Job summary

Evergreen Healthcare Group is seeking a compassionate Clinical Liaison to support the Billings and Laurel markets. You will be the vital connection between hospitals, physicians, families, and our post-acute communities, guiding patients from acute care to rehabilitation and long-term care.

In this regional role, you’ll build relationships with referral partners, evaluate referrals, coordinate admissions, and serve as a trusted clinical resource for patients and families.

Qualifications

• Active LPN or RN license preferred.
• Clinical background in skilled nursing, post-acute care, hospital case management, discharge planning, or healthcare marketing preferred.
• Excellent communication, organization, and relationship-building skills.
• Willingness to travel regularly across Billings & Laurel markets; reside within commuting distance.

Responsibilities

• Build and maintain relationships with hospitals, physicians, discharge planners, case managers, and referral partners in Billings/Laurel.
• Evaluate referrals and assist with clinical screenings and pre-admission assessments.
• Coordinate admissions to ensure timely transitions into skilled nursing communities.
• Serve as a trusted clinical resource for patients, families, and referral sources.
• Partner with facility leadership and admissions to meet census and resident care goals.
• Track referral activity and participate in business development initiatives.

What You'll Do

What We're Looking For

• Active LPN or RN license preferred

• Self-motivated with the ability to work independently across multiple locations

• Willingness to travel regularly throughout the Billings and Laurel, Montana markets

• Must reside within a reasonable commuting distance of the territory
