Job summary

Crystal Creek Post Acute in Stockton, CA is seeking a licensed Physical Therapist Assistant to support Physical Therapists in delivering patient-centered care. You will document progress, assist in treatment plans, and communicate with patients, families, and caregivers.

Requirements include CAPTE-accredited PTA program completion, California PTA license, BLS/CPR, and prior SNF experience preferred. Full-time, day shifts with competitive benefits, EEO employer.

Qualifications

  • Graduate from a CAPTE-accredited PTA program.
  • Active California PTA License and ability to practice in California.
  • Active BLS/CPR certification.
  • Minimum six months of experience in a skilled nursing facility preferred; recent graduates welcome.
  • In good standing with regulatory agencies and licensing boards.

Responsibilities

  • Instructs, motivates, safeguards, and assists patients as they practice exercises or functional activities.
  • Confers with physical therapy staff and others to discuss and evaluate patient information for planning and coordination.
  • Communicates with and instructs caregivers and family members on patient activities and treatment plans.
  • Transports patients to and from treatment areas and secures them into equipment.
  • Administers manual therapeutic exercises, therapeutic massage, aquatic therapy, and modalities including ultrasound.
  • Measures range of motion, body parts, and vital signs to assess treatment effects.
  • Monitors operation of equipment and records its use and patient treatment data.
  • Fits patients for orthopedic braces, prostheses, and supportive devices.

Position Overview:

The Physical Therapist Assistant will help Physical Therapists provide physical therapy treatments and procedures. The Physical Therapist Assistant may, in accordance with state laws, assist in the development of treatment plans, carry out routine functions, document the progress of treatment, and modify specific treatments in accordance with patient status and within the scope of treatment plans established by a physical therapist.

  • Instructs, motivates, safeguards, and assists patients as they practice exercises or functional activities. Observes patients during treatments to compile and evaluate data on their responses and progress and provides results to the physical therapist in person or through progress notes.
  • Confers with physical therapy staff and others to discuss and evaluate patient information for planning, modifying, and coordinating treatment.
  • Communicates with and instructs caregivers and family members on patient therapeutic activities and treatment plans.
  • Transports patients to and from treatment areas, lifting and transferring them according to positioning requirements. Secures patients into or onto therapy equipment.
  • Administers active and passive manual therapeutic exercises, therapeutic massage, aquatic physical therapy, and heat, light, sound, and electrical modality treatments such as ultrasound.
  • Measures patients’ range‑of‑joint motion, body parts, and vital signs to determine effects of treatments or for patient evaluations.
  • Monitors operation of equipment and records use of equipment and administration of treatment.
  • Fits patients for orthopedic braces, prostheses, and supportive devices such as crutches.

Qualifications:

Graduate from a Commission on Accreditation in Physical Therapy Education (CAPTE)-accredited PTA program

Active California Physical Therapist Assistant (PTA) License and is able to practice unencumbered in California.

Active BLS/CPR.

A minimum of six months of experience in a skilled nursing facility setting is preferred; however, recent graduates are encouraged to apply.

Is in good standing with all regulatory agencies and licensing boards.

Working knowledge of Medicare and other payor sources.

Full knowledge of resident’s rights.

Experience in long term care preferred

Able to work on multiple tasks at the same time.
