Job summary

Landis Homes Personal Care seeks a Clinical Coordinator with a strong LPN background to collaborate with the Director of Personal Care and shift supervisors for effective 24-hour management.

You will supervise and train staff, conduct annual One to One conversations, monitor incident reporting, support clinical care, and ensure compliance with policies and regulations, including infection control and HIPAA; on-call rotation as needed.

Qualifications

• Current LPN license in good standing in Pennsylvania required.
• Prior supervisory experience preferred.
• Personal Care Home Administrator license preferred; must obtain within 2 years of hire.
• Knowledgeable of Pennsylvania State Personal Care Home Regulations.
• Proficient in use of computer and related technology.
• Strong verbal and written communication skills.
• Demonstrates leadership, supervisory skills, dependability and a positive demeanor.

Responsibilities

• Work closely with the Director of Personal Care, team leads and alternate shift RNCampus Supervisors for effective 24-hour management.
• Supervises, motivates, trains, and defines the organization's standards and communicates effectively with staff using team leadership concepts.
• Completes One 2 One Conversations with team members annually.
• Relates with other departments and consultants for multi-disciplinary issues.
• Reviews PEER incident reporting and ensures timely follow-up.
• Responds to significant changes in resident condition and provides leadership in emergencies.
• Rounds daily on each house to ensure care needs are met.
• Provides clinical support to the team and oversees quality issues.
• Updates Personal Care Policies annually or with regulatory changes.
• Completes PCQI audits and reports through the PCQI Committee quarterly.
• On-call in emergencies; participates in staffing planning and on-call rotations.
• Participates in quarterly PC Quality Improvement meetings; coordinates monthly billing with Personal Care households.
• Maintains CPR certification.
