Community Home Health Care is seeking an organized LPN to join our nursing department as Clinical Coordinator/Office Nurse. This office-based role supports daily clinical operations, handles nursing calls, assists with referrals and admissions, and coordinates prior authorizations to ensure timely, high-quality care.

The position requires an Indiana LPN license, strong organizational and communication skills, and experience with prior authorizations or referrals is a plus.

Qualifications

• Indiana LPN license in good standing.
• Experience coordinating care and referrals is a plus.
• Strong organizational and communication skills are essential.

Responsibilities

• Receive and respond to nursing-related office calls from clients, caregivers, physicians, and other healthcare professionals.
• Assess concerns within the nurse’s scope of practice and communicate urgent matters to clinical leadership.
• Submit prior authorization requests with Medicaid, insurers, and other payers.
• Follow up on pending authorizations and troubleshoot delays, missing information, denials, and other issues.
• Maintain accurate authorization records and communicate approval status to nursing, scheduling, and billing staff.
• Assist with admissions and take new referrals for the nursing team.
• Review required clinical and referral documentation.
