Job summary

Landis Homes Personal Care is seeking a Clinical Coordinator (LPN) to lead the Personal Care team under the Director of Personal Care, ensuring 24-hour management and highest care standards.

You will supervise staff, conduct One-on-One conversations, oversee incident reporting, ensure regulatory compliance, and support dementia care training. Pennsylvania LPN license and supervisory experience are required or preferred.

Qualifications

• Current license in good standing as an LPN in Pennsylvania.
• Prior supervisory experience preferred.
• Personal Care Home Administrator license preferred, must obtain within 2 years.
• Knowledgeable of Pennsylvania Personal Care Home Regulations.
• Proficient in computer use and related technology.
• Good verbal, non-verbal and written communication skills.
• Demonstrates leadership, supervisory skills and dependability.

Responsibilities

• Works closely with the Director of Personal Care for 24-hour management.
• Supervises, motivates, trains, and sets standards with staff using team leadership.
• Completes one-to-one conversations with team members annually.
• Relates with other departments and consultants on multi-disciplinary issues.
• Reviews PEER incidents daily and ensures timely follow-up.
• Responds to significant changes in resident condition and leads during emergencies.
• R rounds daily on each house to meet care needs.
• Provides clinical support to team members.
• Oversees quality issues and follows up on incidents.
• Updates Clinical Personal Care Policies as needed.
• Completes PCQI audits and reports quarterly.
• Maintains time management and accuracy in duties.
• Monitors infection control issues with the LH team.
• Promotes Personal Care concepts and resident independence.
• Assists with direct care when needed and participates in activities.
• Meets with families with leadership input and reports concerns.
• Acts as a change agent and communicates changes positively.
• Ensures compliance of RASP's and DME's.
• On-call in emergencies; shares on-call rotation ~every 4 weeks.
• Coordinates staffing with scheduler to cover call-offs.
• Participates in quarterly PC Quality Improvement meetings.
• Coordinates monthly billing assessments and submits to Finance.
• Maintains current CPR certification.
• Relates well with residents, family, staff and community.
• Adheres to dress code and Honoring Lives leadership principles.
• Supports growth and development opportunities for team members.
• Communicates calmly and with patience and sensitivity.
• Completes Dementia Friendly training within first year.
• Engages in training opportunities and helps schedule training.
• Demonstrates dependability, competence, accuracy and vigilance.
• Adheres to confidentiality and HIPAA; complies with safety and infection control.
• Understands Code of Conduct and compliance guidelines.
• Additional duties as assigned.
