Optimal Care is seeking a Clinical Lead to partner with the Regional Practice Manager and oversee providers across post-acute care settings. You will be an expert in Physician Services EMR and internal systems, guiding visits, care settings, and service lines to meet quadruple aim goals.

Responsibilities include onboarding new providers, managing orientation, and strengthening facility relationships to grow the practice. Strong clinical leadership is essential.

Qualifications

• State certification and degrees need to practice medicine as a Nurse Practitioner.
• Current State License as a Nurse Practitioner.
• 2 years clinical experience in a post-acute care setting.
• Ability to practice medicine skills as defined by accepted licensure boards of Nurse Practitioner.
• Demonstrates ability to assess and respond to the needs of patients, families and caregivers in post-acute care settings.
• Demonstrates skill in practice of medicine, ability to cope with family emotional stress and tolerance of individual lifestyles.
• Ability to handle multiple priorities, documentation requirements and deadlines.

Responsabilités

• Oversee providers within the Practice as part of the Practice Dyad.
• Be an expert in Physician Services EMR and internal operating systems.
• Guide visits notes and care settings; ensure service line rules and pathways.
• Share responsibility for productivity of the Practice provider group.
• Develop orientation plans for newly hired providers.
• Oversee orientation for newly hired providers with Regional Practice Manager.
• Assist facility relationships to grow practice or support service recovery.
• Advance Practitioners within the collaborative/Practice Agreement.
• Ensure organizational initiatives are implemented in a timely manner.
