Generation Clinical Partners is seeking an experienced nurse practitioner or physician assistant to join our post-acute and long-term care practice in Missouri. The role focuses on high-quality, cost-effective care for frail elderly patients in skilled nursing facilities.

You will lead care coordination, contribute to quality improvement, and collaborate with physicians in a flexible, technology-driven environment with a cloud-based EHR and home call rotation.

Qualifikationen

• Missouri RN/NP or PA licensure is required.
• MSN or equivalent advanced degree preferred.
• National Board Certification strongly preferred.
• Minimum 2 years of experience in acute-care, long-term care, or inpatient rehabilitation settings.
• Demonstrated interpersonal and collaborative skills.
• Ability to adapt to practice schedule and ensure high-quality patient care.

Aufgaben

• Provide direct patient care and coordinate care transitions in skilled nursing facilities.
• Lead quality improvement initiatives and ensure cost-effective treatment plans.
• Order and interpret diagnostic tests, prescribe medications, and manage chronic conditions.
• Educate patients and families on self-care, health promotion, and disease prevention.
• Collaborate with physicians and other staff, with possible referrals as needed.
• Provide clinical leadership and staff education to improve facility performance.
• Document evaluations, diagnoses, procedures, and outcomes in the EHR.
• Participate in call rotation from home with collaborating physicians.
