Optimal Care seeks a Clinical Lead Nurse Practitioner to oversee providers within the Practice Dyad and to deliver high-quality medical care across post-acute settings in collaboration with the Regional Practice Manager.

The role focuses on EMR expertise, physician services, and enterprise-wide improvement while maintaining patient-centered care and ensuring smooth onboarding for new providers.

Qualifications

• Active state license as a Nurse Practitioner.
• 2 years clinical experience in a post-acute care setting.
• Ability to practice medicine per licensure board standards.
• Strong assessment skills for patients, families, and caregivers in post-acute care.

Responsibilities

• Oversee providers within the Practice Dyad to ensure quadruple aim.
• Serve as expert in Physician Services EMR and internal operating systems.
• Guide visit types, notes, and care settings for providers.
• Share responsibility for provider productivity with Regional Practice Manager.
• Develop orientation and onboarding plans for newly hired providers.
