Job summary

Pasadena Post Acute is seeking a dedicated LPN/LVN to provide direct nursing care to residents and supervise nursing assistants, ensuring compliance with state and federal regulations. You will lead shift operations, review care plans, document thoroughly, and communicate with physicians, residents, and families to maintain high-quality care and safety.

CPR certification and an active license are required. This role requires strong leadership, teamwork, and the ability to manage multiple tasks

Qualifications

• Must possess a Nursing Degree or be a graduate of an approved LPN/LVN program.
• Current CPR certification and an active license to practice as an LPN/LVN in the state.
• Ability to read, write, and understand the English language.

Responsibilities

• Direct the day-to-day functions of the nursing assistants.
• Admit, transfer, and discharge residents as required.
• Chart nurses' notes and ensure accurate documentation.
• Maintain the Daily Census Report and submit to the Business Office as required.
• Participate in the facility’s quality assurance program and surveys.
