Job summary

Strategic Interventions, LLC is seeking a Clinical Assessor to manage the intake and referral process for adults with Severe and Persistent Mental Illness across North Carolina. You will review eligibility, conduct screenings, and coordinate admissions with the ACTT/FACT teams, ensuring timely access to services and maintaining confidential, person-centered care.

Qualifications

• Active North Carolina licensure as a mental health professional.
• Minimum of two years of experience working with adults with Severe and Persistent Mental Illness (SPMI).
• Experience conducting comprehensive clinical assessments and intake/referral processes.
• Knowledge of community-based behavioral health services and recovery-oriented care.
• Strong clinical judgment and ability to identify urgent behavioral health concerns.
• Excellent communication, interpersonal, and customer service skills.
• Ability to work collaboratively in an interdisciplinary team.

Responsibilities

• Receive, review, and track referrals from individuals, families, providers, hospitals, and community partners.
• Gather demographic, clinical, and referral information and obtain required documentation.
• Conduct initial clinical screenings and identify urgent concerns requiring intervention.
• Coordinate eligibility reviews, program fit assessments, and admission decisions with clinical leadership.
• Complete Comprehensive Assessments for ACTT-referred individuals.
• Schedule intake assessments and ensure required consents and paperwork.
• Maintain accurate referral logs and intake documentation in designated systems.
• Communicate professionally with members, families, referral sources, and treatment staff.
• Facilitate smooth transitions into services and provide community resource referrals.
• Collaborate with Team Leads, the Clinical Director, and interdisciplinary team members.
• Participate in meetings, training, and quality improvement initiatives.
