Sevita's South Carolina Mentor is seeking a Family Preservation Case Manager to coordinate case management and counseling services for youth in Chester, Lancaster and York Counties. You will assess referred individuals, develop and monitor service plans with treatment teams and funding sources, maintain accurate records, and provide ongoing mentoring support, including visits and crisis response.

A Master’s degree in human services is required and licensure (LBSW/LMSW/LCSW) may be needed.

Qualifications

• Master’s degree in human services is required.
• Two years of related experience with individuals who have developmental disabilities or mental health issues.
• Licensed or license-eligible (LBSW, LMSW, LCSW or LCSW-C) if required by state.
• Valid driver’s license with access to a vehicle if transportation is provided.

Responsabilités

• Provide case management and counseling services to an assigned caseload.
• Assess referred individuals and lead development, coordination, and evaluation of service plans.
• Maintain current records in compliance with standards and regulations.
• Provide ongoing monitoring and consultation to mentors, including home visits and group meetings.
• Participate in emergency coverage and crisis intervention as needed.
• Support licensing, regulatory activities, and build mentor networks.
