South Carolina Department of Social Services in Darlington County seeks a CWS Case Manager - Family Preservation to provide time-limited, safety-focused interventions and collaborate with families to ensure safety, permanency and well-being. The role emphasizes empathetic engagement and coordinating services within the GPS Practice Model.

The position requires a Bachelor's degree in Social Work, Behavioral Science, or Social Science, with options for substitution by higher degrees.

Formación

• Bachelor's Degree in Social Work, Behavioral Science, or Social Science.
• Bachelor's Degree in any field with one (1) year experience in a related field; Master's Degree may substitute for required work experience.

Responsabilidades

• Provides case management in the Child Welfare area of Family Preservation per DSS Policy and GPS Practice Model.
• Performs on-call duties as assigned and maintains caseload safety, permanency and well-being.
• Completes and updates FAST to collaboratively develop Family Permanency Plans and make timely service referrals.
• Engages families and professionals with empathy and respect; responds to calls, emails, and texts promptly.
• Maintains case records and documentation per policy; ensures quality standards in CAPSS system.
