Acentra Health invites you to join as a Clinical Evaluator (PASRR) in Nebraska. You will conduct PASRR Level II assessments across hospitals, nursing homes, and community centers, guiding level-of-care decisions to support quality care and regulatory compliance.

This PRN hybrid role offers the flexibility to work from home while traveling locally for in-person assessments; travel expenses are reimbursed and the company provides the necessary technology and equipment, including a company-issued

Qualifications

• Must hold an active, unrestricted Nebraska license in a healthcare or related discipline, including but not limited to LCSW, LMSW, LMFT, LICSW, LCPC, LCP, RN with Psychiatric Specialty, or other LMHP.
• RN candidates: Current, unrestricted RN licensure required. No minimum degree beyond licensure education.
• Behavioral Health/Mental Health candidates: Master's degree in a behavioral health or related field is required.
• 3 years of experience in assessment/diagnosis of mental or psychiatric disorders, including complex differential diagnoses.
• Minimum of 2 years of experience with intellectual/developmental disabilities, serious mental illness, or related conditions.
• Proficiency in medical record abstraction and review.
• Strong clinical judgment, evidence-based assessment, and critical-thinking skills.
• Excellent interpersonal, verbal, and written communication skills.

Responsibilities

• Complete PASRR Level II assessments by reviewing records, gathering information from collateral sources, and evaluating physical, behavioral, and psychological functioning.
• Develop informed level-of-care recommendations and determine the most appropriate next level of care for individuals who may require long-term care placement.
• Review and verify medical records and provide clinical recommendations per regulatory requirements, including evaluating psychiatric input and physician notes.
• Interact with healthcare professionals, patients, their families, and internal partners to support the assessment and review process.
• Leverage technology and proprietary software to efficiently perform job functions and manage assessment activities.
• Ensure timely assessments within scope of practice to meet contractual and program requirements.
• Read, understand, and adhere to HIPAA and related privacy and security policies.
