Job summary

Talent is seeking a Patient Enrollment Manager to serve as an embedded presence at partner clinics across the area. You will engage prospective patients, explain the program, and guide them through enrollment, working directly with care teams to ensure a smooth patient journey.

This role requires being on-site and traveling to multiple clinics, with an emphasis on patient education, care navigation, and building trust in a fast-moving startup environment.

Qualifications

• Hands-on clinical or patient-facing background, e.g., nurse, PT, PTA, MA, health coach, or similar.
• Comfortable in clinical environments: exam rooms, care teams, direct patient conversations.
• Experience in patient education, care navigation, or health coaching is a plus.

Responsibilities

• Serve as an embedded presence at one or more partner clinic locations.
• Engage prospective patients, introduce the program, and guide enrollment.
• Educate clinic staff and support new partner sites as the program scales.
• Collaborate with care teams to ensure a seamless patient experience.
• Maintain accurate records of interactions and enrollments.
