Job summary

The U.S. Department of Veterans Affairs, Fort Meade, SD, is seeking a Dietitian (Clinical) to provide Home Based Primary Care. Responsibilities

Conduits comprehensive nutrition assessments using the Nutritional Care Process (NCP) and nutrition-focused physical examinations to establish a nutrition diagnosis and determine risk.
Develops and implements Medical Nutrition Therapy (MNT) to address the nutrition diagnosis and monitors outcomes.
Calculates nutrient requirements and tailors diet prescriptions to individual patients, considering food and drug interactions.
Documents MNT in the medical record using the ADIME format.
Functions independently with accurate record keeping and Interdisciplinary team communications.
Identifies patients in need of nutritional support and communicates recommendations to the health care team.
Provides nutrition education/counseling tailored to Veterans and uses virtual care to improve access.
