Job summary

The U.S. Department of Veterans Affairs seeks a Dietitian (Clinical) providing Home Based Primary Care (HBPC) within the Clinical Section of NFS to serve as the medical nutrition expert.

Dietitians coordinate nutritional care of Veterans with other health care providers, Veterans, caregivers, and the community. They are autonomous in applying evidence-based nutrition guidelines for patients with complex needs, delivering care using the NCP (assessment, diagnosis, intervention, monitoring and

Qualifications

Knowledge of accrediting and regulatory requirements.
Knowledge of virtual technology to provide nutrition care.
Ability to deliver patient care using the NCP (assessment, diagnosis, intervention, monitoring and evaluation).
Ability to apply evidence-based nutrition practices to improve patient outcomes.
Ability to develop patient-centered goals.
Ability to collect data for performance improvement studies.
Ability to collaborate with stakeholders.

Responsibilities

Provide HBPC nutritional care coordinating with Veterans and care teams.
Apply evidence-based nutrition practices to complex cases.
Develop and monitor patient-centered nutrition goals.
