Job summary

The Dietitian (Clinical) provides Home Based Primary Care (HBPC) within the Clinical Section of NFS, serving as the medical nutrition expert. They coordinate nutritional care of Veterans with other health care providers - the Veteran - the Veteran's caregiver - and the community.

They are autonomous in applying evidence-based nutrition practice guidelines for patients with complex medical and nutritional needs.

Qualifications

• Must be US citizen.
• Proficient in spoken and written English.
• Credentialed or eligible for RDN/RD per VA policy.

Responsibilities

• Conducts nutrition assessments using the Nutritional Care Process (NCP).
• Develops and implements Medical Nutrition Therapy (MNT).
• Documents MNT in medical records using ADIME format.
• Communicates with health care team and Veterans to tailor nutrition care.
• Uses virtual care technology to improve access to care.
