Qualifications

• Registered by the Commission on Dietetic Registration.
• State license in good standing with active status.

Responsibilities

• Assess residents' nutrition needs and document plans in EMR.
• Develop and evaluate individualized care plans based on data.
• Provide nutrition education to residents, families, and staff.
• Collaborate with interdisciplinary teams to improve health outcomes.
• Ensure CMS/state regulations and company standards are met.
• Provide coverage across buildings based on need.
