Job summary

St. John's Home in Rochester, NY is seeking a Clinical Coordinator to support and lead nursing staff, ensuring care plans and assignments enhance resident rehabilitation outcomes.

The role collaborates with medical providers, aids staff development, and oversees clinical operations with a schedule Monday-Friday days and every other weekend required. Pay is commensurate with experience; generous extra pay for hours after 3:00 pm and on weekends.

Qualifications

• Current RN with a minimum of one year in long-term care or acute care.
• Minimum of one year formal leadership experience in health care.
• Rehabilitation experience is highly desirable.
• CPR certification is required.
• Excellent communication skills.
• Willingness to support and lead others to embody St. John’s Brand Characteristics.
• Compliance with NYS immunization requirements.

Responsibilities

• Coordinate clinical services for each resident.
• Oversee development of individualized care plans.
• Generate nursing staff assignments with clear written and verbal communication.
• Create a compassionate environment with emotional, psychological, social and spiritual support.
• Assist floor nurses with bedside care and perform rounds; support Nursing Department audits.
• Ensure staff embraces and lives the St. John’s Brand Characteristics.

Position Summary:

The Clinical Coordinator is responsible for supporting and leading dedicated nursing staff . The Clinical Coordinator ensures staff assignments provide care that enhances residents' ; collaborates with medical providers; assists with staff development; and oversees clinical operations leading to positive rehabilitation outcomes.

The schedule for this position will be Monday-Friday on days with every other weekend required.

Pay is commensurate with years of clinical and leadership experience. Generous extra pay for hours worked past 3:00 pm and on weekends.

Qualifications / Education:

• Current RN a minimum ofone (1) year of long term care or acute care experience required
• Minimum of one (1) year formal leadership experience in health care required
• Previous Rehabilitation experience highly desirable
• Current CPR certified required
• Excellent communication skills
• Willingness to support and lead others to living St. John’s Brand Characteristics
• Compliance with NYS Department of Health immunization requirements.

Physical Requirements:

• Sits, stands, walks most of the day. Frequent moving and lifting of residents. Moderate to heavy lifting (up to 50 lbs).

• May be exposed to infection, odors, psycho-behavioral situations.
