St. John’s Home in Rochester, NY is seeking a Clinical Coordinator to support and lead nursing staff. The role focuses on care quality, staff collaboration with medical providers, and overseeing clinical operations.

The schedule is Monday-Friday days with every other weekend. Pay reflects experience, with additional pay for after-hours and weekends. This is an equal opportunity employer.

Qualificações

• Current RN with at least 1 year of long-term care or acute care experience.
• Minimum 1 year formal leadership experience in health care.
• Rehabilitation experience highly desirable.
• CPR certification required.
• Excellent communication skills.
• Willingness to support and lead others to living St. John’s Brand Characteristics.
• Compliance with NYS Department of Health immunization requirements.

Responsabilidades

• Coordinate clinical services for each resident.
• Develop individualized care plans.
• Generate nursing staff assignments with clear written and verbal communication to support resident care needs.
• Establish a compassionate environment by providing emotional, psychological, social and spiritual support.
• Assist floor nurses with bedside care, perform rounds, and assist with Nursing Department audits.
• Ensure staff embrace and live the St. John’s Brand Characteristics in daily duties.

Position Summary

The Clinical Coordinator is responsible for supporting and leading dedicated nursing staff . The Clinical Coordinator ensures staff assignments provide care that enhances residents' ; collaborates with medical providers; assists with staff development; and oversees clinical operations leading to positive rehabilitation outcomes.

The schedule for this position will be Monday-Friday on days with every other weekend required.

Pay is commensurate with years of clinical and leadership experience. Generous extra pay for hours worked past 3:00 pm and on weekends.

Qualifications / Education

• Current RN a minimum of one (1) year of long term care or acute care experience required


• Minimum of one (1) year formal leadership experience in health care required


• Previous Rehabilitation experience highly desirable


• Current CPR certified required


• Excellent communication skills


• Willingness to support and lead others to living St. John’s Brand Characteristics


• Compliance with NYS Department of Health immunization requirements.

Physical Requirements

• Sits, stands, walks most of the day. Frequent moving and lifting of residents. Moderate to heavy lifting (up to 50 lbs).

• Responsible for coordination of clinical services for each resident
