Job summary

Community Home Health Care seeks an organized LPN to join our nursing department as Clinical Coordinator/Office Nurse. This office-based role supports daily clinical operations, handles nursing calls, assists with referrals and admissions, and coordinates prior authorizations to ensure timely, high-quality care.

The position requires an Indiana LPN license and preferred home health care experience. You will interact with clients, families, physicians, and payer representatives while maintaining

  • Current Indiana LPN license in good standing.
  • Home health care experience preferred.
  • Experience with prior authorizations, referrals, or clinical coordination preferred.

Responsibilities

  • Receive and respond to nursing-related office calls from clients, caregivers, physicians, and other healthcare professionals.
  • Assess concerns within the nurse’s scope of practice and communicate urgent or unresolved matters to the appropriate clinical leader.
  • Apply for and submit prior authorization requests with Medicaid, insurance companies, and other payers.
  • Follow up on pending authorizations and troubleshoot delays, missing information, denials, and other authorization issues.
  • Maintain accurate authorization records and communicate approval status, service dates, and authorized hours to nursing, scheduling, and billing staff.
  • Take new referrals and assist the nursing team with the admission process.
  • Collect and review required clinical and referral documentation.
  • Maintain tracking systems for supervisory visits, comprehensive assessments, recertifications, and OASIS documentation.
  • Provide timely information and updates to case management staff.
  • Monitor missed-visit reports and assist with required physician notifications and orders.
  • Communicate and cooperate with other departments to support clients and employees.
  • Maintain accurate and timely documentation in the agency’s electronic medical record system.
  • Follow HIPAA requirements and all applicable federal, state, payer, and agency regulations.
  • Assist other departments and complete additional duties assigned by management that are appropriate to the employees’ training, licensure, and skills.

Qualifications

  • Knowledge of medical terminology.
  • Strong organization, communication, follow-up, and problem-solving skills.
  • Ability to manage multiple priorities and meet important deadlines.
  • Comfortable communicating by telephone with clients, families, staff, physicians, and payer representatives.
  • Experience with electronic medical record systems preferred.
Work Environment
  • Office-based nursing position.
  • Requires prolonged sitting and regular computer and telephone use.
  • Must be able to communicate clearly, maintain confidentiality, and work effectively in a busy environment.
