A leading home health provider in Orlando, Florida, is seeking an LPN Office / Clinical Coordinator to ensure effective patient care coordination and assist with office tasks. The ideal candidate will be a licensed practical nurse with excellent customer service skills, the ability to handle stress well, and familiarity with office software. Responsibilities include documentation, coordination of patient care services, and participation in HR activities. Bilingual candidates are strongly preferred.

Qualifications

  • Active Licensed Practical Nurse with a current unencumbered license in Florida.
  • Current CPR certification required.
  • Experience with Office products including Excel, Outlook, and Word.
  • Valid driver’s license and auto insurance.

Responsibilities

  • Ensure effective patient care coordination and office coordination.
  • Participates in onboarding and clearance procedures.
  • Document communications and notes from the patient's chart.
  • Prepare and manage documentation for nursing and office processes.

Position Scope:

To ensureeffective patient care coordination/human resource/office coordination needsfor the services working with the company's personnel, and through liaison withother organizations and individuals also providing care to the patient/client.

Ongoingreviews the utilization and progress of the patient with the RN, supervisor andattending physician as necessary.

Lines of authority and reportingresponsibilities: Report to the Director of Nursing, Clinical Manager, NurseSupervisor, Administrator.

Position Qualifications:

Graduateof high school or with some years of high school education (no Diplomarequired). Evidence of knowledge of home care environment; ability to workunder stress, and to take rapid actions.

Verbaland written communication skills.

Isresponsible for confidentiality issues.

EnsuresHIPAA guidelines and procedures are maintained.

Understandsthe nature and type of the patient/client population serviced.

The LPNwill be aware of the responsibilities of all organizations and individualsinvolved in patients/clients care/service including the coverage for theservices rendered.

The LPNwill participate in education conferences, meetings, in-services and trainingfor policy and procedure modifications, emergency response and preparednessplan, organization planning, quality assurance and company activitiesimprovement, etc.

The LPNgives accurate information to clients, clients families and other professionalsinvolved in patients care/service.

Complieswith all applicable policies and procedures, federal and state rules,regulations and laws in effect.

Participatein personal growth and development.

Documents/filesall communications and the communication notes from the patient's chart.

Required Qualifications:

Active LicensedPractical Nurse with a current unencumbered license in the state of Florida

Have andmaintain current CPR certification

Possess a validand current drivers license and auto insurance

Experience withOffice products including, but not limited to, Microsoft Office, Excel, Outlookand Word document. Willingness to learn other office and software programs

Electronic MedicalRecord system familiarity

  • On-call rotation participation as indicatedby business needs

Preferred Qualifications:

BilingualSTRONGLY preferred (English and Spanish)

Tech Savyhighly desired
